Complaints

The department investigates all allegations of licensing violations in child care facilities. The Department also investigates individuals who are allegedly providing care for more than 2 without a license.

Complaints related to a child care facility can be registered in one of the following methods:

When making a complaint against a child care facility, when possible, please include the following:

  • Name of facility
  • Address of the facility
  • Person filing the complaint (Names are kept confidential)
  • A contact telephone number
  • The date and reason for the complaint.

If you are unable to leave your contact information, please contact your local DFS office or Child Care Licenser to report your concerns.
